After August 1, 2021 all schedule change requests will be reviewed and processed through the attached form. Please complete this form to request a schedule change. Forms may be mailed to Ursuline or dropped off during normal business hours. Mrs. Carosella (email@example.com) will follow up by phone or email with the necessary updates.
Courses will be changed for the following conditions: there is an error in the original schedule, the student is academically misplaced, or there is a scheduling conflict among chosen courses. A $25.00 fee is assessed for any changes in a student’s schedule that do not meet the above conditions. The administration reserves the right to deny a schedule change request based on class size, graduation requirements, appropriate academic placement, or other scheduling issues.
